HUD: OK to Use Expiring Forms, for Now
Several HUD forms are expiring at the end of March 2014. Although using expired forms can result in a Management and Occupancy Review finding, you won't be penalized for using these forms until the new ones are issued. These forms include model leases, 50059s, Race/Ethnicity Form, EIV Coordinator Access Authorization Forms and User Access Authorization Forms, and some sample forms.
According to HUD, new forms with new expiration dates have been submitted to the Office of Management and Budget (OMB) for approval. And OMB has advised HUD that if new forms are not released before the expiration date, the current forms are valid until new versions are posted. The following HUD forms expire March 31, 2014:
- All HUD Model Leases (90105A, 90105B, 90105C, and 90105D)
- Form HUD 50059 – Owner’s Certification of Compliance
- Form HUD 50059A
- Form HUD 27061-H – Race & Ethnicity
- Form HUD 90100 – Recertification Notice
- Form HUD 90101 – Certification of Long Term Care Insurance
- Form HUD 90102 – Sample Verification Of Disability When Eligibility For Admission Or Qualification For Certain Income Deductions Is Based On Disability (202/8, 202 PAC, 202 PRAC, and 811 PRAC)
- Form HUD 90103 – Sample Verification Of Disability When Eligibility For Admission Or Qualification For Certain Income Deductions Is Based On Disability
- Form HUD 90104 – Sample Exceptions to Income Limits
- Form HUD 90011 – EIV Coordinator Access Authorization Form
- Form HUD 90012 – EIV User Access Authorization Form
- Form HUD 91066 – Certification of Domestic Violence, Dating Violence or Stalking
- Form HUD 91067 – VAWA Lease Addendum
